IMPORTANT:  THE FOLLOWING TERMS AND CONDITIONS GOVERN THE CONTRACT BETWEEN ALL CLIENTS (INDIVIDUALLY) AND REVIVE SKIN & AESTHETICS (THE MED. SPA / AESTHETICS CLINIC) FOR PROVIDING FACIAL, SKINCARE AND OTHER AESTHETICS SERVICES TO CLIENTS PURSUANT TO A BOOKING.  YOUR ENJOYMENT FOR YOUR MEDICAL SPA EXPERIENCE IS IMPORTANT TO US.  PLEASE READ THESE TERMS AND CONDITIONS CAREFULLY TO ENSURE YOUR COMPLETE UNDERSTANDING AND AGREEMENT.  IN CONSIDERATION OF THE BOOKING CONFIRMATION AND/OR VOUCHER ISSUED TO YOU BY THE REVIVE’S WEBSITE AND/OR ANY THIRD PARTY LIKE LAZADA, ETC., ALONG WITH OTHER GOOD AND VALUABLE CONSIDERATION, ALL CLIENTS HEREBY AGREE TO THE FOLLOWING TERMS AND CONDITIONS (“CONTRACT”):

Quality Assurance

We are committed to ensuring that our products and services are of a satisfactory quality and in compliance with the Sale of Consumer Act of the Philippines (Republic Act No. 7394) (Consumer Act).

Definitions

Products” shall refer to any tangible merchandise offered by us for sale, including but not limited to facial, skin, and body-related products. 

Services” shall refer to any hands-on treatment rendered by our medical aestheticians and therapists, including but not limited to facial and skin care treatments.

Warranties and Service Guarantees

 

Products: We provide a one-week defect-free guarantee for all products. We will replace the defective product with an identical product if it is proved to be defective, provided that the defective product is returned to us within one (1) week from the date of purchase, together with the relevant receipt. Where we are unable to replace the defective product with an identical product, we will refund the amount paid by the customer in full within two (2) weeks from the date the defective product is returned.

Services: Given that the efficacy of our facial treatments, skincare and aesthetics services may vary depending on the individual, including one’s physical/or and mental state of health, we are not able to guarantee any desired results or outcomes from our treatments.

Health, Allergies or Pregnancy Considerations

The treatments and services provided by us are intended only for general wellness purposes and are not intended to substitute professional medical treatment for any condition, medical or otherwise, that clients/patients may have. 

Clients/patients with any existing medical conditions are advised to consult their doctors before seeking or receiving any treatments or services from us.

 

You are required to disclose in your booking request if you have any health conditions, limitations, allergies, physical ailments and/or if you are pregnant, and you will notify our medical easthetician and therapist immediately of any discomfort experienced during your treatment.

 

You agree to fully indemnify and hold harmless The ReviVe Skin & Aesthetics, its affiliates, subsidiaries, representatives, agents, and employees, from and against all liabilities, claims, expenses, damages and losses, including legal fees (on an indemnity basis), arising out of or in connection with the products and services provided to you.

Payment Methods

We accept the following methods of payment for our products and services:-

* cash (PHP)

* bank transfer / online banking

* g-cash

* maya

* coins.ph

* 7-11

* Credit card payment

       An electronic receipt acknowledging payment will be issued to all clients/patients within (24) twenty-four hours from the day of payment.

 

 

Booking Policy 

We require a P500 to P3,000 Security Deposit for all clients. This deposit will be deducted from your total bill when checking out. If you need to cancel, your deposit is 100% refundable when you follow the ReviVe Invoice Terms & Conditions. 

Require Reservation Deposits:

*  P500 to P3,000 (depending on the treatment)  Security Deposit for ALL                new clients. 

*  P1,000 to P3,000 (depending on the treatment)  Security Deposit required              for existing clients who have violated our Cancellation Policy.

*  P500 Security Deposit required for existing clients

*  Please pay the Invoice before the payment due date to confirm your                   

      appointment.

* Appointments with non-paid Invoices may be canceled at our will.

Check-in
We ask that you please arrive 10-15 minutes prior to your appointment time. We will do our best to accommodate late arrivals. However, the length of service may be adjusted so as to not interrupt the scheduled appointments of other guests. The full price of scheduled services will apply, so please plan accordingly. Of course beyond the check-in policies, we invite all our clients receiving services to make full use of our relaxing environment here at ReviVe Skin & Aesthetics.

 

E-mail Reservation Services

We offer E-mail reservations as a courtesy to our clients. E-mail reservations will follow all other Policies and Cancellation Policies. We will need just a few client details for our records in order to proceed further to secure your reservation.

 

Please provide us with the following: 

- Full Name 

- Contact Number

- Interested Service / Treatment

- Date / Time

 

After we have received this information, we will further coordinate your booking. We will require a P500 to P3,000 Security Deposit for all first-time E-Mail Reservation users, sent by ReviVe or PayPal invoice in order to secure your reservation, a PayPal account is not needed to make payment.

 

Please Note, this invoice will need to be paid within 30 minutes of delivery to reserve your appointment/s. If your appointment/s is on the same day as your E-Mail Booking, we will need immediate payment. If payment is not received after this time, we will cancel your reservation, and all future bookings will be mandatorily done over the phone at 0930-461-2299 / 0906-184-0007. 

Deposits are 100% Refundable when you follow our ReviVe Terms & conditions. 

 

New Clients

All new clients will be subject to a P1,000 to P3,000 (depending on the treatment)  Security Deposit for their first visit. After this visit, you will be on an honor system. If you fail to follow our Cancellation Policy as an existing client you will be subject to another P1,000 to P3,000 Security Deposit. 

 

Please arrive 15 minutes before your scheduled appointment in order to fill out any necessary paperwork, settle in, or use the restroom. Your appointment may be canceled if you are more than 30 minutes late without calling the clinic to notify us that you are running late. Any appointment thereafter, we recommend coming in 30 minutes early.

Existing Clients

Existing Client will be required to provide a P500 Security Deposit to confirm any appointment requests, sent via ReviVe or PayPal E-mail Invoice. Please pay by payment due date to confirm the reservation. A PayPal account is not needed to make payment. 

 

Discounts

ReviVe Skin & Aesthetics often offer discounts, please mention your discount at check-out. Discounts cannot be combined, discounts are not applied to "Packages".  Please visit the "Hot Deals" page on this website to view specials promos.

 

We offer a 10% discount to ReviVe’s VIP Members, please present your active card at check out. Our services are set at a reasonable and competitive price for all!

 

Gift Certificate Policy

Gift Certificates are available in any amount and may be purchased at the clinic / medical spa, by phone at 0930-461-2299 or you may contact us below.

 

Out of town? Too busy? No problem. We will mail your gift certificate to you. Gift certificates cannot be redeemed as cash. Gift Certificates must be mentioned when making your appointment.

 

ReviVe's Promotions 

ReviVe's Specials cannot be used in combination with any other offer, promotions, or third-party gift certificates. 

 

Regulations

Cell Phones & Use: Please turn off your cell phone or turn it to vibrate upon entering the spa. Please try to have phone conversations before or after your appointment in the lobby area, out of consideration for other clients who come to Island Skin Spa to relax, and would be disturbed by noisy ringing or conversations. 

Children: For safety reasons and the relaxation of all clients, children who are in car seats, strollers, or need to be held ARE NOT permitted in the treatment rooms and may not be left unattended in the waiting area.  Older Children ARE NOT allowed to be in Treatment Area for appointments over 15 minutes. We have the right to refuse service if we feel the need to ensure the tranquility of the clinic. 

 

We do not encourage extra people in the Treatment Area other than the person receiving the service. 

 

Pre- Appointment Cancellations and Rescheduling

You may cancel or reschedule your booking at any time before it has been confirmed. Once confirmed, you may only cancel or reschedule up to twenty-four (48) hours before your appointment is scheduled to begin without any penalty.

For any cancellation or rescheduling between twenty-four (24) hours and four (4) hours prior to the scheduled appointment, 50% of the collected deposit will be forfeited and you will be required to top-up the deposit before we reschedule another appointment.

For any cancellation or rescheduling less than six (6) hours prior to the scheduled appointment, 100% of the collected deposit will be forfeited and you will be required to pay another deposit before we reschedule another appointment.

Please notify us via email at reviveph.clinic@gmail.com  or call/txt at            0930-461-2299  or  0906-184-0007 to cancel/reschedule your appointments, this must be done 48 hours before your scheduled appointment time to SAVE your 100% Deposit.

Your Reservation Deposit will be forfeited if you "No-Show" (fail to arrive without notification) or "Late Cancel" (Please give us ample time to adjust or cancel your appointments.

For example, if you book a Facial (60 minutes) you have up to 1 hour BEFORE your scheduled appointment time to notify the clinic of your cancellation to avoid a "Late Cancel". In certain conditions, a 50% deposits for "Late Cancels" will be saved to your account for your future use. If you have any other questions or need these policies verbally explained, please feel free to give us a call at 0930-461-2299 / 0906-184-0007.

Exchanges and Refunds

For single treatments:

As it is our policy to confirm the treatment(s) and price(s) upon the receipt of the deposit and prior to the commencement of treatment, we strictly do not entertain any requests for exchanges or refunds once treatment has begun.

 

For prepaid package and promo package treatments:

We may entertain requests to exchange prepaid packages at our sole discretion. In the event that the existing package is to be changed to another of a higher value, you will be required to top-up the price difference. We do not provide any refunds for the exchanging of an existing package for another of a lower value. If the prepaid package has been partially used, the value of the existing package will be determined by deducting the regular ala carte price of the service(s) already consumed.

We provide a one-week cooling period from the date of purchase for our customers to seek a full refund of payment should they wish to cancel their prepaid package treatments, subject to the following conditions:

      * We do not provide any refunds after the expiry of the one-week cooling period.

 

Transfers

We may entertain requests for the transfer of prepaid package and promo package treatments (or any unused balance thereof) at our sole discretion, subject to the provision of full and accurate information in relation to the proposed transferee.

Transparency and Accuracy of Pricing

All prices for our products and services are net prices and will be clearly communicated to our clients/patients. Additional charges such as add-ons or other services will be clearly reflected as separate and distinct from the prices of the main service(s). The validity of promotional prices and discounts will be clearly communicated to our clients/patients.

We are committed to ensuring that all prices quoted at the time of booking are honored, even if later adjusted for inflation and other costs.

 

 

Data Security

We will procure your consent prior to any use of your personal information for any marketing purposes.

Changes to Terms and Conditions

We keep the Terms and Conditions under regular review and reserve the right to revise these Terms and Conditions at any time in our sole discretion without any prior notice.

The date these Terms and Conditions were last revised is set out below. You will be deemed to have consented to and be bound by any amendment to these Terms and Conditions when you purchase our products or engage our services on and from the effective date of the last revision.
